Overview
The Oklahoma Academy of Integrated Care Conference will feature speakers with qualified experience in Integrated Care models and a keynote speaker who is nationally known for his work in Collaborative Care.  The two-day conference will offer two tracks focusing specifically on pediatric and adult practices to target the specific needs of our participants.  It will also feature an “Integrated Care” workshop where participants will be able to participate in a hands on activity simulating the work done in the Collaborative Care Model, an evidenced based model with over 77 randomized control trials showing effectiveness in treatment.  Additionally, Oklahoma Health Home agencies will be present to educate providers of their services, as well as networking with providers in the area. 

Objectives

Upon completion of this activity, participants will improve their competence and performance by being able to:

  1. Describe the need and function of a variety of integrated care models.
  2. Apply their knowledge towards implementation of integrated care models in their own practice.

Accreditation

The University of Oklahoma College of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The University of Oklahoma College of Medicine designates this live activity for a maximum of 12.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.


The Osteopathic Founders Foundation is accredited by the American Osteopathic Association to provide osteopathic continuing medical education for physicians. OFF designates the Oklahoma Academy of Integrated Care Annual Conference for a maximum of twelve and one quarter (12.25) AOA Category 1-A credits. The Osteopathic Founders Foundation will report CME for all osteopathic physicians who complete and return both an attestation form and program evaluation for all sessions attended. Credit will be commensurate with the extent of the physician’s participation in this activity.
