Emergency Response with Dementia Adults, Course #26D37 - 6/10/2026

The ECHO format is a clinical case presentation by a community support professional team member, followed by a 25-minute informational presentation delivered by a guest speaker and a 15-minute QI by the ECHO Hub Team. One session on six topics offered two days a week for six weeks.

1. Identify and integrate evidence-based interventions that prevent functional decline and promote quality of life among older adults in long-term care settings.
2. Evaluate residents’ complex medical, cognitive, and psychosocial needs to develop coordinated, interdisciplinary care plans that align with individual goals and preferences.
3. Implement practical strategies to enhance communication and collaboration among healthcare team members, residents, and families to ensure consistent delivery of age-friendly care practices.
