Overview

This intensive one day offering that includes a review and update on major subject areas in rheumatology. The faculty chosen to speak are recruited locally and from nationally recognized leaders in their fields.  The topics are chosen from the prior years participant suggestions.  The seminars are focused on practical diagnosis, treatment, and latest research in areas related to the successful delivery of rheumatology health care and patient outcomes. The University of Oklahoma College of Medicine faculty is nationally recognized for their expertise in the management of complex conditions that is unavailable elsewhere in the state, region or sometimes even the nation. This is a very fast-paced symposium with five lectures and concurrent “meet the professor” sessions, conducted in a small-group discussion format. Each session will include time for questions and answers. The course combines best practice strategies and education through case studies and lectures.

Overall Professional Practice Gaps:
Gap 1:
 Pregnancy in rheumatic conditions can lead to poor maternal and fetal outcomes. Health care providers may not be aware that evidence-based guidelines to manage pregnancy in rheumatology conditions have been recently published entitled: 2020 American College of Rheumatology Guideline for the Management of Reproductive Health in Rheumatic and Musculoskeletal Diseases.
Gap 2: Lupus nephritis is difficult to treat but guidelines are available as well as new treatments. Physicians may be unaware that in 2019, the joint guidelines for the management of adult and pediatric lupus nephritis were updated by the European League Against Rheumatism and European Renal Association-European Dialysis and Transplant Association (EULAR/ERA-EDTA). 
Gap 3: Practitioners vary widely in their screening, surveillance, and treatment of lupus nephritis (LN).
Gap 4: Practitioners may desire additional information about the two recently FDA approved treatments for lupus nephritis and other therapies in the pipeline.4
Gap 5: Diagnosis and treatment of the inflammatory myopathies continues to be a challenge. Presentation may be heterogeneous and extra-muscular features may dominate the clinical picture. Practitioners may not be aware of the current knowledge of the clinical associations and the possible pathogenetic implications of the myositis autoantibodies, which is needed for optimal clinical use of these autoantibodies.
Gap 6: Practitioners may not be aware of the different methods used to test for myositis autoantibodies, which could affect results and clinical implications. Understanding the differences can help in interpretation of autoantibody tests.
Gap 7: Physicians may be unaware of the multitude of rheumatic complications occurring after COVID-19, including long COVID, and vaccine responses on rheumatic medications, and outcomes of rheumatic patients that get COVID-19. 
 
Gap 8: A positive antinuclear antibodies (ANA) can be found in up to 25% of women in the United States and a positive ANA remains as one of the most common reasons for referral to a rheumatologist. Gaps exist in: Identifying the features of ANA positivity which are associated with development of a systemic autoimmune rheumatic disease.|
Gap 9:  Identification of environmental factors that contribute to autoantibody positivity in individuals who do not yet have a systemic autoimmune rheumatic disease. 
Gap 10: Evaluation of high-risk factors for disease transition to classified autoimmune rheumatic disease in individuals with undifferentiated connective tissue disease.


Objectives
Upon completion of this activity, participants will improve their competence and performance by being able to:

  1. Describe how pregnancy outcomes in lupus data was assessed and interpreted.
  2. Demonstrate how to implement pregnancy guidelines in the clinical care of patients.
  3. Estimate risks of medication use during lupus pregnancies with risks of under treated disease.
  4. Summarize the temporal changes in pregnancy outcomes in SLE as management paradigms have evolved.
  5. Describe lupus nephritis screening and surveillance guidelines.
  6. Identify lupus nephritis treatment guidelines.
  7. Examine two FDA approved therapies for lupus nephritis: Belimumab (Benlysta), Voclosporin (Lupkynis).
  8. Discuss potential future lupus nephritis therapies.
  9. Describe the clinical associations of myositis autoantibodies.
  10. Recognize the characteristics, advantages and disadvantages of different testing methods for myositis autoantibodies.
  11. Use myositis autoantibodies in the diagnosis and classification of patients who may have myositis.
  12. Evaluate whether people receiving immunosuppressive drugs are more susceptible to initial infection with SARS-CoV-2.
  13. Predict the outcomes of COVID-19 in immunosuppressed individuals.
  14. Discuss clinical management in patients with autoimmune diseases, including COVID-19 vaccine efficacy and treatment and prevention of SARS-COV-2 infections.
  15. Identify autospecificities that can assist with identifying individuals with low risk of serious autoimmune rheumatic disease
  16. Utilize currently available testing to evaluate environmental risk factors for transition to SLE and other autoimmune rheumatic diseases in autoantibody positive individuals (with or without UCTD)
  17. Discuss autoantibodies which are commonly found in post-acute sequelae of COVID19, along with associated rheumatic disease symptoms
  18. Describe the earliest clinical and serologic features of systemic lupus erythematosus (SLE)
